This app has made it easily to solve authenticating possibly fraudulent orders. We use their email sign up form, but converted to a verification form and it has been perfect. Very easy to use and the best looking of all the custom form apps we tried. Highly recommend.

Needed a simple to create/use form app that created a nice looking wholesale customer registration form for our website.
This did the job perfectly.
Until we realised that the County(State) hard coded options were incorrect.

Their support team jumped straight on it and within a few hours had fixed the issue and updated the entire UK county options.

The app does exactly what it should!

Some minor adjustments were needed and I was able to do most of them myself.

At some point I needed a small adjustment in the app and they added the feature within a day!

I used to use the Shopify form. But they don't have the features I need, like auto approval, customer tags and redirect to login.
This app is very good. I created custom forms for 2 of my B2B stores. It's easy to create, customizable and meets all my needs. I contacted support once for a customization and got a response in a second! They get the job done very quickly. I recommend this app!

This app is exactly what I needed, and functions just as well (if not better) than all the other more expensive apps I’ve tried. I had a lot of trouble setting up my email notifications to my customers with the app for some reason, but customer service did it for me so fast!
