Needed a simple to create/use form app that created a nice looking wholesale customer registration form for our website.
This did the job perfectly.
Until we realised that the County(State) hard coded options were incorrect.
Their support team jumped straight on it and within a few hours had fixed the issue and updated the entire UK county options.

Great product, direct integration with shopify and directly integrates with account verification emails sent through shopify.
