Zotabox Contact Form Builder

I set this up as a simple contact us form and it works. The setup was easy. I like that you can pick your own background, use your own artwork.

Awesome app. Lightweight, seemless integration and fully customizable, Place the "Contact Us/Message Us/etc" tab anywhere on your site, even in the header or footer menu. Customize the tab and the contact form to match your store. Thanks guys.
