Very simple and easy to set up. We are now using it in both of our Shopify shops. I love that I can use my own background image and customize the colors to match my stores.
So far so good. Seems really easy to use and has a bunch of other built in options that may eliminate other apps we're running.

This is a very easy to use and professional looking Contact Form! I also recommend the Easy Popup and Promo Bar apps as well. I had no problems creating a Contact Form or using the app. It's a very simple, quick and impressive solution to generating a forms page!
