We installed this form app because the previous one we had been using was no longer supported. This one is super easy to install and works better than the previous app.

Happy with the app. Hopefully one day we can allow more than 20mb, but this appears to be a Shopify limitation, rather than the app's.
I must say, Qikify is very good at responding to support.

Frustrated that yet again Shopify doesn't include some very basic functions within the platform but this app made creating a multi field form very easy. Also, the support from the team was excellent, to resolve some styling issues that we had.
Would recommend this app.
