We have been using this app for some years and it is great. We recently had an issue where it stopped working, so we contacted support and they were very fast in finding and resolving the issue. I would recommend this app to anyone :)

Very easy to setup and works perfect. Nice templates. It would be nice if you could use links in field names, e.g. in the privacy checkbox. How can I activate Custom Fields as a required field?

We setup this contact form as a means for people to contact us for wholesale orders. It is extremely user friendly and easy to configure. You can customize with drop down menus as well. We had some difficulty having it only pop up on one of our menu tabs and not all, but contacted customer service and they helped to remedy the issue right away. Highly recommend!
