This app has the functionality you need for an easy to use and form which can be embedded easily by pasting some code in a page using the editor button. They reply and act swiftly on support questions so if you are looking for a tool to implement form of any kind, this is the one you want!

So far it's pretty easy to set up. I just set up the form so for now I will give it 5 stars, then come back and update if not working right.
