I needed a good simple contact form with Google CAPTCHA and tried Simple Contact Us Form.
I have a premium template for my store, and the contact form did not automatically work so I got in touch with support.
My experience with the support was nothing short of perfect.
They got back to me fast, Their developer went into my code, fixed everything up and its all working perfect within 24hours.

Great App. Great usability. And Super fast first level support! Can recommend. I just had a quick error, that the form wasn't displayed and the support was able to give me direct and fast (less than one minute) reply with instructions + video. And that worked pretty well!

I am NOT tech savvy at all, but I managed to install app with a little patience, and from viewing the two videos very carefully. Have a little refining to do. Waiting on response from tech support.
