Great app, works as intended. I was able to add custom forms to my checkout page to gain additional info about my customers.

So far so good. I like all the options that it provides and how specific you can be. If it works as advertised it will be a great asset.

I have spent the last two week trying to find a way to make a few text boxes mandatory on my cart page. I found this app. and it does exactly what I was looking for.

I've just downloaded this app so that my customers could select multple delivery times to receive their orders. So far so good. Seems quite easy to use. I just wish that the information would be added to the order confirmation email from Shopify.
