Looks good so far - Very eager to get rating but it does what I need by the looks which is great.

The navigation is a bit clunky but this app works well and is simple enough to use.
I would like to see a few more options for the type of attribute.

This is a great app, have been looking for a way to gather more information about my customers, wanting to personalize our engagements.

Gives us the additional form fields we need. Works great! Excellent customer service! Only downside is the inflexibility of the form. If you change the fields, you loose the content of the old fields.
