App works exactly as you would imagine. It is also useful for tweaking some of the customer data points Shopify does not give and we need to have from our customers. I would like to give a special shout out to Brett as he was very helpful showing a lot of professionalism.

Super easy to set up and customize, streamlines info inputs for my different customer groups, and as the owner of a small business offering a range of resin jewelry supplies that are specialized by project idea, this app has been helpful in segmenting my users and get them the inspiration they need to keep coming back. Very prompt support and great customer service. I had spent a bunch of time trying to figure out how to do something, and eventually gave up and asked them if there was a way to do it, and BAM they made the change for me within an hour!
Update 2023: Still excellent, and new features since my initial review in 2018 have allowed me to do lots of new things. 6 stars!
Very quick at answering any questions I have! Very easy to build your form with whatever fields you want. Also, they are very nice people to work with!

Quick tip: You may want to consider adding a separate form for retail customers as well. On your current plan, you can add an additional registration form at no additional cost. We're standing by to help if needed 👍
