Helium's team is top notch!! We created a Form through them and we were having issues with the email flows that we set up on our own. When I reached out to the support team (specifically Brett), he got all the way into the email template code through Shopify and helped me figure out exactly what the issue was. We are using Helium Customer Fields along with its sibling app: Strike Automatic Discounts.
Both are great app, Strike to offer automatic discount, Customer Fields to set up a form.
The support is responsive and friendly, very easy to understand without cumbersome technical terms.
They are open to new ideas and needs of merchants, too. So we are really happy.

The best app for creating custom sign up forms. Support is top tier. If you need the best and don't mind paying a little more than alternatives, Helium is the way to go!
