Finally found something that just works, after months of reading forums and trying dozens of hacks with no results - this app "just works"

Have been using this app for years. Then when Shopify went to the new Checkout Extensibility, it broke - pop-ups can no longer be used unless you revert to the old order status page, which is only a temporary fix. This updated app uses a different method to show the opt-in, and works with the new Checkout Extensibility pages. Support, if or when you need it, is excellent. Worth every penny.

We were using another provider and the opt in survey suddenly stopped working so none of our customers were able to leave reviews.
