Very good App and special thanks for ankit he deal very friendly.

The free version lets you create a form, but does not allow you to see the results when the form is completed. You have to upgrade, which of course makes the "free" plan useless.

I like the app but don't like the constant pop up to give it a rating after telling asking not to show again.

Very easy to install and create the forms that I wanted. I was up and running in no time. Support is very helpful if required.

simple and good, did exactly what it wanted. tested a few apps and this is the winner. also great customer service btw!
