I tried several forms and this one was the easiest to create and have it integrate perfectly with my site without many limitations (like others).
There were a few configurations I had trouble with on my own (dropdowns popping up based off a selection and auto responder) but their technical team fixed both of my issues promptly.

This app works great and lot's of great features to build any form you need to run your business. Customer service is one of the best I have seen. Had a small issue, response time in chat was very quick and they solved my issue within minutes with their development team.

We started using this app in June 2018 and it has been the sole form provider for all of our forms. When any of our apps are filled out and submitted it sends an email notification to our support desk and we are able to manage from there.
