This app is great for contact pages. I highly recommend to anyone looking to offer file upload options. The support is extremely helpful and took care of all issues I encountered.

UPDATE from ~6 months in: Still 5/5 stars. I've reached out to support for the odd question here and there, and every time I've gotten a response within *minutes*. And it's not just an auto-responder. It's a knowledgable, helpful person every time. Can't say enough good things about this app.
Original review: I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need).
The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections.

I used this app to create a "Get Involved" form. It's free, and it works great. My favourite feature is that you can have some questions only appear on the form if the visitor selects a specific option from a drop down menu. Also, you can have an automatic response sent to the customer from your email address when they fill out the form - so they know that the form went through. No complaints here - great options for a free app.
