I had issues at first, however the staff was extremely responsive and helped with providing the solution I needed to make the form work exactly how I wanted within the store!

This app is easy to set up and use. I did need some help from support but it was quick! My problem was solved in under 5 minutes.

The UI looks really good and easy to setup setup forms. Captcha without all the google setup. You can customize fairly easy. Forms have a simple div tag you add to the code or the page editor.
