The Hulk form creator is quite simple to use. It generates a really clean appearance and very usable form for nearly any type of form you can think of. I'm using it for a brand ambassador application and it has every feature I needed and more. I needed help with the app integration to my theme, so I opened their live chat and within an hour of telling them what I wanted, they do that for me.
And they helped me to where I struggle. I would like to thank the developers and team have done a wonderful job.

Very capable Form Builder app. Easily the most flexible and powerful form builder in the Shopify App store. Free level is good but very limited in features. My only complaint is that we signed up for Pro level - and even at that level there are some email design features that they block out unless you go to the very top level. That is in my opinion very unfair and a trap for young players.

App is super simple to use. I had a little hiccup getting the form to show properly but the rep Sanjay was super helpful!. I got disconnected from our chat midway thru and had to start a new chat, Deepa was able to get me transferred back to Sanjay just in time for him to fix the issue. Great teamwork and support! thanks again
