We use this app for our online forms and it's easy to set use. The tech support is helpful and quick, although the time zone difference delays things more than we'd like. We're in the US.

Great visual appearance and easy to use. Support has been responsive when we have had issues with our host receiving notifications, and helped us solve the issue.

useful so far although its a bit annoying when using on mobile - doesnt seem very responsive - also it could do with more options like time slots for appointments etc

This app has good basics, but for our purposes there are not nearly enough design customizations available.
