The best app out of there, tried different and this have everything you need. If there isn't something, the support can help and code for you a solution. Before buying try it for free, because everyone have different needs and what for me was good maybe is not good for you!

I am a fan of Globo. I spent days looking for an app or places that allow me to do forms and no luck. I end up finding an app that I can work with and was happy about that. THEN......I've found Globo "Powerful Contact Form Builder". Sad to say, I dropped the first app that I used and now is using Globo. Why do I do that? I got to say, their work and customer service is excellent!!!!

Absolutely perfect APP for collect customer informations on your store. The form has lots of functions and you can custom it by yourself, if you still have other requests, just contact online cha, the customer service will get back to very soon and they will try to fulfill your needs! I like this app and will install it on my rest of stores!

I used this app and it was pathetic to its core. I had to rebuild my whole story because this app messed up my theme code and i feel sick to my stomach to say this but i just want you guys to be aware of this app and be very very careful.

I just wanted to add a simple contact collection form for a freebie download. It was easy to create and add to a page in Shopify. It also automatically integrates contacts into my customers list for email marketing. I did need to email tech support to customize my fonts but they were so helpful and did the work really quickly (and for free). I have the free version so it does display "powered by GloboForm" but I don't mind since this is for a one-time Zoom event and not my main newsletter form.
