I have tried multiple survey form app and Powerful Contact Form seems to be the best & easiest to use. To publish the form on a page, simply paste the form code on a new Shopify page. Love it.

The app is working well, and I got a good support as well! I had a small issue that I could not save the change I made, so I contacted the support, and they responded pretty quickly. Ultimately, I just needed to update the app, but I just couldn't figure it out how to do it. They sent me screenshots, videos, and reference pages so that I can fix the issue. Even though I'm using the free plan, the support is stellar!

I've been using the free version for quite a long time. It's really great and very customizable. I needed help after my theme update and Emma from the support team was very helpful and she got back to me immediately. There are many other apps from Globo and they all are very good and the support team is always quick and helpful.
