I use this app to get feedback from my customers. Its a great free app that works perfectly for my store. It has different formats that we can chose from and the interface is quite simple and user friendly.

We've been using this app for about a year. It's so easy to set up a form with it. The rare times I needed any support the Globo team have very very helpful, responsive and courteous. They really seem to want to do right by the customer.

This app can handle a LOT of submissions. One of our promotions went Viral and we had 30,000 submissions in just a matter of a couple of hours. This app handled the work load with little to no problem. It is a great way to have your Shopify forms send to different emails without having to deal with a lot of coding to make it work.

We use the forms in several sections of our store. They work great and the help we get from the support team is also fantastic.
