When our reg page stopped working after we implemented a new website design, we reached out to the Form Builder team. They immediately responded to our help request and made sure our webpage was back up quickly. We are very happy with the app and the team's fast response when a problem happened. I would recommend this product to others.
We've been using the app on two stores for about 18 months or so and it does everything we need it to do, one site sends our contact form to Zapier to open a Zendesk ticket, the other site uses it primarily to directly send data to a Google Sheet. We had built out a very nice multi-step form as well for one site but it was for a short-term project. All not typical usages and many more integrations are possible, it does way more than we need. Support has been excellent, responding quickly to the one or two times we've needed to reach out.
Used this app successfully for several months before switching themes when it stopped working. Contacted support and had the issue resolved with a few hours, thanks guys!

This app works really well. We've been using it for years with no issues and one day there was a problem with my email rejecting the form notifications. I contacted support and they responded immediately and fixed the issue. Highly recommended!
