We've been using this app to gather specific information we need from customers when placing orders that Shopify doesn't automatically gather, and we've loved how much we can customize it to meet our needs. We had some issues with a recent Shopify theme template upgrade and when I reached out to customer service, they responded within 5 minutes and fixed the issue. Super impressed!
Quick to install, super easy to use with many customization options. Support staff are very prompt in replying to our queries. Highly recommend.
