We love this app for creating more customized forms. There are many templates to choose from, even on the free plan. Lots of customization options and it's basically plug and play creating a new form. Highly recommend if you are looking for forms that go beyond the basic Shopify offering!

I have used many contact form apps, and this one is by far the best that I have found! The monthly expense is well worth it compared to others!
