So far enjoying using the form builder, I am hoping to be able to make use of the free plan for our needs, but not sure if that is going to be possible yet.

Excellent full-featured app with highly customizable settings. Forms can include virtually every data entry field. Many templates or build your own from scratch. Your forms will look great.

This is a great app. The plans are not very expensive and the app is easy to use. Ive tried others but this is the best one I could find.
