Great to create nice easy forms for your customers to use. You can design it and have enough options for the free plan. Would recommend!

Love this app. Easy to understand and utilize. Makes things easy for anyone to list products and go add space for customers to put in their personalization information.

So far I really like it!! I like that it is easy to create the forms. Image upload is great! You do have to upgrade for that feature but the pricing is fair.
Great & cost-effective service. Quick and easy, nice templates. Great starter option for anyone looking to add a basic contact form

Great app, does exactly what I need.
I'm surprised that this app is free (or at least that the free version is so useful). It automatically sends me an email when someone fills out a form and I have a folder that the emails automatically get forwarded to. Lots of people visit my the form and it doesn't break (as far as I know). Also asked customer service to fix an issue with the display on my website and they fixed it shockingly fast. Highly recommend this app if you need a form builder.
