This app did exactly what I wanted it to do. The layout was simple and quick and will save me so much time! I wish I had found this earlier.
Even after opting for paid subscription the form still displays their advertisement at bottom. The ticket has been created on this but no response has come. Its been more than 24 hours. Worst support.
This app does exactly what it says and works very well! I would recommend this app to anyone that is looking for a GREAT contact form.

Simple easy to use app, wanted to create a registration form for resellers. The great features allowed me customize the form to the required information. Also has pretty useful design tools.
