I love how you can create your own fields in the form. Very easy to use and install to my shopify store.
Answered my questions very quick and promptly! For some reason the photo was not looking right on mobile and they resolved this for me within minutes. Thank you so much!

I did this app to help me create a form for a potential job applicant. it had all the fields I needed. But I wished it had the feature to add a sliding scale for getting feedback.
I really liked how it was really easy to add fields for the form I was creating. I would have liked it better if it had some options to add music in the background.
