So Far so good with this app, it has helped me with creating a form so my clients can fill out for my custom diet and training program.

really easy form builder.... little confusing on how to make the form for only certain parts of shopify
I used this app to help me get an email list started for my new store. I really liked how easy it was to use and design my form. I hated that I couldn't link MailChimp on the free version. It said MailChimp on the app name, so I assumed that was included. It should serve my purpose, though.
Super easy and friendly to use. My only complaint is that I cannot add suggested content to my form field.
This app was super easy to use. I wish that the width of the form could be a bit wider and adjustable to the Shopify width of the page.
I used this App to set up a question form. Super helpful Go to zetuafrica.org to signup for our Kickstarter launch via the form.

But would have given 5 stars if the form has a preloader.
I used this app to build my contact form on my online website. It's so easy to use and I was able to edit everything I need to change to make it my own. Would recommend!
