Incredibile add-on for Shopify!
Perfect! Super!
I am using this to take custom surfboard order inquiries. This will help get contact information but i want to be able to offer a downloadable/printable order form but havent figured that out yet

First time user. Very intuitive and quick to build on. Would recommend it to anyone wanting quick and easy form building.
This app had some great features, but ultimately didn't help us with variables in merchant billing. Also, most importantly, the loading speed is significantly slow. Are there any ways around this?

I used this app to install a custom order form section in my web store. It was easy to use, and maybe I missed it, but I would like to be able to add pictures to the custom form. Overall, great, and I will use to create again!

This may be one of the best apps in the Shopify app store. I wish I would have found it sooner. Great customer service and a lot of options.
