So far, so good! I just used this app to set up a sample request form in about 20 minutes. For a free app, this thing can do pretty much anything. I absolutely love the Mailchimp integration - exactly what I needed! I would love the functionality to be able to subscribe these users to a specific segment on my Mailchimp list. But, super happy with this app so far!

This app has been incredibly useful for gathering information from customers as well as creatively using the features to avoid having to add additional plugins.
