OVER CHARGING ISSUE! BE AWARE!!
I installed this app on 3 stores, and after the trial period, I went ahead and upgraded the plan in one of the stores. Instead, they charged me the upgrade in 2 stores out of the 3.
When contacted the customer service after 2 min it happened to tell them about the issue, they said they can't return the money they took with no permission, and they can only cancel future payments.
Since then a week past no reply
Super simple and easy to use. Made it very easy to add to my store. Highly recommend using this for your forms.
I found this easy to use and very customizable for my needs. Also so easy to install on my shopify page.

I added this app to my contact page to streamline inquiries. I love the editor and found so many advanced features that I wasn't expecting. My form is fairly simple, but I was really happy with how customizable this form builder is. I've set an autoresponder so that people submitting an inquiry can get an immediate confirmation, and I've customized the message that displays after submission. I was able to customize all the fonts and colors, and even the style of inputs. So far, this form builder seems to have all the functionality I could need. I'm starting out with a fairly basic form, but I saw some interesting features like file upload elements and limiting submissions. I can see myself using this form builder if I need one for custom order forms, raffle, or event sign up in the future. So glad I found this!

Form Builder is very easy to use and has helped to organize some of our incoming information quite a bit.
So far i really like this app and used it to create my feedback and vendor forms. Its easy to use as well!

Has everything you need to build a form and more, saves you hours in coding them yourself.
We needed a larger form so can get quite expensive.
