One of the highest quality apps I have ever used on Shopify. It has immense flexibility (more so than any paid versions I have seen), and a really polished user interface. It is truly astonishing that it is free. I have built dozens of Shopify stores and would recommend to anyone from a developer to a novice.

I used this app to do a simple quotation form. Has saved me time, money and was so easy to use. Can't wait for my clients to use it!

Easy and quick to use. Versatile. 5 stars

I used the app to building a form to register warranties for my products. It was easy to use and gave me just the options I needed. However, once I spent them time to build the form and tested it, it never would send the form info to the email that I provided. Tried many times. Then discovered that it will only take up to 25 form inputs per month. Since I built that for a Warranty Registration, I'll go through 25 in no time. Not as useful as I had originally thought. Bummer

Really easy app to use! Quick and simple to install - going to be really useful on our site. Integrates fully with MailChimp and easier to customise than their forms!
