After looking through the App Store for a survey app, Form Builder was the easiest solution compared to the others I installed and tried out. It had all of the functionality that I needed and made building my form so simple, with great customization abilities. If this app could get any better, I would prefer to pay a one time charge per form rather then being billed ongoing for hosting a survey on my site, and sometimes when I do a test run on the survey page, it runs slow to load which I worry will detour interested people. That aside, all is great and this is the app I would recommend as the best choice for Shopify and surveys!!
Free version is great for a simple "contact me form" but Shopify already does this. Was looking for more element options than 5 in free version.
It is not free as it claims to be. The free version is so limited that it is simply not worth the while to build a form and be faced with tonnes of restrictions.

Just installed this app yesterday, made a form in five minutes, super easy. I will be upgrading to a paid plan to get more bells and whistles, although the free plan has a lot of features, I want more so my form looks awesome. Love that it integrates with shopify.
This app is very intuitive. When I had a problem the app developer answered incredibly quickly and seemed genuinely concerned about my problem. I really liked that and wish him thanks. Definitely a useful app to have with a lot of support and instructions to use it easily.

I used the app to collect registration information at the time of paying fees. I really like how the forms data can automatically sync to Google Docs.
I used this app to create an application form on my Shopify store. It is the only one I found that had the conditional logic I needed. It's fairly easy to use and did what I needed. Thanks!
