Fantastic app for building forms with lots of options for customizations. Integrates nicely with Shopify.

Have been using for years, very good app. it provides all the functions we need, and very easy to build.

This is a really great app! Does exactly what I need for forms for our website! I use it for multiple forms and they are are all used daily! I highly recommend this app.

Excellent app so far – very customizable with lots of features, even allows our visitors to upload photos from their desktops or mobile devices. We haven't tested out the rest of the features yet, but highly recommended so far.

Best form builder I used by far for shopify. I have tried them all, and this one is the easiest to use.
