App is perfect for making any forms you need on your site! I used it to make a application for my brand reps
This app was exactly what I needed for my business it was easy to add field to the forma and adding it to website only took 1 click.
The app is great, however, it had one issue that made us spend a lot of time fixing it. Which shouldn't had happened in the first place.

I really loved this app. Just had a hard time figuring out making it work when it was already published on my site but nonetheless, looks like it's easy to figure out. :) Great app
It just perfectly meet our needs. No need to add code in the background. Installation was quite easy, automatic.

It was fairly easy to create and install. The only thing I'm not happy about is that is that it is very slow and when someone goes to my page, you see the code for a second and then the form appears and the code is below the form. It looks unprofessional so I'm trying something else
Easy to use! I had my form made and published in minutes, I have only had my online store for a week and this has been the easiest part so far!
