Sooooooo Easy, and just enough options for a nice, clean, and simple form!
Excellent app that helped me achieve professional looking custom forms. I am now about to have customers submit artwork for a quote!
This app is great so far. I'm using to capture leads at an exhibition for a free giveaway. I would definitely recommend for any forms.

I used the app to building a form to register warranties for my products. It was easy to use and gave me just the options I needed. However, once I spent them time to build the form and tested it, it never would send the form info to the email that I provided. Tried many times. Then discovered that it will only take up to 25 form inputs per month. Since I built that for a Warranty Registration, I'll go through 25 in no time. Not as useful as I had originally thought. Bummer
Really easy app to use! Quick and simple to install - going to be really useful on our site. Integrates fully with MailChimp and easier to customise than their forms!

I love this app. I created for a booking form and it was so easy to use. I love that the app automatically installed it for me without the need to input a embedded code.
