POWR Form Builder Contact Form

I used this app to help me create a free questionnaire form. Very simple and easy to use, and very easy to install into your Shopify store.

I used this app to help me design a form to accept custom design request. It took a minuet, but was pretty easy once I got the hang of it.

Instructions on how to rearrange form elements could be a little more prominent.

This app works ok. The Business Plan is too expensive, but the customer service team helped us find a more affordable plan for my business' needs.

Very simple and straightforward. I like to add the div to either a semantic modal or just plop it in my footer. Great app.
