I purchased the pro version out of necessary as my previous contact app I was using stopped working several times and I needed something fast. It is an easy to use app with basic features for creating and installing forms on a website. Nothing fancy... The plus to this app is it's easy to use and installs the script where ever you want it on any pages you create. It has basic design elements. The downside is that for the cost... it is too basic. I would like to have more options for designing forms and adding elements that are not available in this app but are available in others. Overall I am happy so far as it does what it says will. I would give 5 stars except I feel it is over priced for what it does.

It's basic but gets the job done. I used for a custom order form and had the choise between listing everything in one section, or within sections and conditional display.
I have been wanting to add a lead page to my site and Form Builder made it very easy to do so. Simple intuitive interface, hoping to grow my list as a result!

It was easy to set up and use POWr forms on our site. We are using the limited free version, but pay for the Map. Both work well for the simple forms that we need, including payment.
So far so good! Lots of options here! I still have yet to see how all of the settings will work out with my current store but it certainly looks promising.
So far it is great even in the free version. Intuitive, customizable, flexible and very easy to use...bravo!
This app is really easy to use and they have WONDERFUL support!
Richcarde has been super helpful on their support team! Two issues I have are: 1) I wish there was a bit more customization available, and I wish the form loaded faster on our website (it's always the last element to load on the page).

Solve my problem and I'll change to 5 stars.
Problem: The only poor thing is that it loads a bit slow on my page, but that is maybe because I have the PRO version + many questions and options due to selling customizeable products.
Otherwise, the app really helps me build forms to retreive valuable information from my visitors.
Its flexible and works well.
