Really liked this app! Was simple and easy to maneuver through. 10/10 would recommend for any shopify user.
Form is perfect. Juwan in the support department got me squared away with all of my questions on the conditional formatting as well. This goes great with the Plan Comparison app also.
Easy to use and templates are ready to go already which made it so much easier! The first template was glitching though, but I think it may have been my connection.
