does it has a feature of sending fromm submission to different admin base on different subjects selected

So far so good! Loved the options! I used my form for a Athlete submission form. Seams to have all the areas I needed thanks!

Update: it's been 6 months. I have tried to edit the form and it saves but does not re-publish the saved form....Will have to dig deeper.

Good app. Really easy to install and get set up. I had a booking form for my bike service business up and running in just a few minutes. Really flexible options and extremely intuitive to use. Only two things I would have liked to see:
1) It would have been handy if I could limit the date & time options to keep them within my businesses working hours. At the moment with this form it would be possible for someone to book a bike in for 11:30pm on a Saturday night (when I am most definitely not open for business)
2) There are a few fields that have the option of being set at 'half-width'. But if I do that, it would be good if there was also a way of setting the next form element to sit next to it (i.e in a two column grid).

I used this app to build a form in 10 minutes with no experience with this POWr or Shopify. After a long time of research - seemed to easy to just plug it in.

Pretty simple and easy! I built a pretty complex form in a matter of minutes!! Once built, I was able to test it and send information directly to me. I love how you can export the information to Excel to get a quick list of customers who have completed the form! Again, simple and easy to use!!

We just started using Form Builder. The app is an easy way to build your network and audience. Great stuff!

So far I'm loving what I'm seeing. Very easy to use. Tested it. Works great. There are very smart premium features that work with Google Sheets and text alerts. I will probably upgrade to gain access to those features. I will say that the free account comes with distracting and even confusing messages that appear after a form is completed. Confusing to the form completer that is. When I did a test run, I saw a confirmation message "Received, thank you". However in larger more attention grabbing letters directly below, it read "This form was created with POWr. Create Your own" which was underlined and hyperlinked like a call to action. Nobody completing a form needs to be presented with a new hyperlink upon clicking "Submit." Anybody intelligent enough to need an online collection form will see subtle branding and know how to track down the company behind an amazing form experience they just had. However, the average online form filler outer may "not" be that web savvy and may think they have to take one more step to complete the form. I do not like that, so I'm only giving a 4/5 star rating.

Easy to use and integrate to the website, also very easy to edit and change the way it looks. The only reason for the 4* is that it takes quite awhile for the code to load, and for a few milliseconds you can see the code on the page! However does the job it's suppose to and works well enough. I have had many customer inquiries using the form which gets sent directly to my email. You can find an example on my site on the main menu..."More", and then "Corporate Order Form".

I used this app to create a booking form for workshops in my store. Ii is great how customizable it is. I wish I could add different payment options for different bookings.
