Setting up is easy. Since its a free native app from Shopify, I believe it will already be optimized for performance. A lot of merchants have requested for options to customize designing, and Shopify has assured it is in the works. I have a few suggestions:
1. Give option to add separator line between sections
2. Give option to add a different email id for each submission to be sent to, instead of registered email id.
3. Allow exporting submission entries on demand
4. Allow making headers bold
5. For single-line and multi-line text fields, allow adding headers
6. Allow splitting form into multiple pages
7. Include "OTHERS" as a default option for radio buttons where needed, and when selected, give an option to include a field for users to add text

Team Wet and Dry Personal Care
At first this app was working great for me, the flow of the forms was exactly what I was looking for. It was sort of a hassle linking it on the pages I needed them on but once I figured it out it looked great. I don't know for how long this has been occurring, because it used to work, but I checked my website on Safari and the multiple forms I had made do not show up on the Safari browser (I checked on multiple devices and no result). When I checked on Chrome they did show up. I am unsure if other people have had this issue, but I'm sure a majority of my traffic uses Safari and will not have access to the forms that would help me as a business tremendously.

Covering many basic properties of forms functionality it does allow to collect data. But the lack of making translations to the fields or changing styling is despite of many requests in forums a bit of a statement of not listening to urgent customer needs or thinking functional scenarios completely through.
Setup was easy and most customers use it. However, I have two suggestions:
1. Add the ability to insert an open text field to remind customers to confirm acceptance in their email.
2. Modify the birthday field in the data download. The pop up appears correctly, but the customer data download reads yyyy-dd-mm for some customers and yyyy-mm-dd for others.

They fixed the bug where you cannot uncheck required so updating to 3 stars.
Very limited customization and no easy way to customize css do to its use of a custom element and a shadow DOM, so you cannot just add css to control the form and any form you make will not match your existing website css but will have its own css that cannot easily edit.
There also appears to be a bug where you cannot uncheck required for First name and Last name fields.

Currently, adding css customizations isn't possible. This is an area we've heard feedback around and are looking at ways to improve the design flexibility of forms. For now, customizability is limited to the options in app.

It's OK for a very basic use especially for start ups. However disappointing that extraction of the data captured is not possible unless you install another 3rd party app which you might need to pay for.
Hi there! While exporting data directly from metaobjects (where the data is stored) is not yet possible. You can install Flow and use the "export to google sheet template" https://help.shopify.com/en/manual/promoting-marketing/create-marketing/forms-app/flow.
Flow is a Shopify app and is free to use.

Unbelievably useless App. Sure, it collects data and collects it in Shopify, but it's metadata, which is inaccessible by external no-code tools like Make/zapier. Regardless of what you do the apps cannot read the metadata within Shopify to extract data to tools like Airtable, which are vastly superior database tools.
Shopify decided to make it nearly impossible for non-coding people to use form submission data outside of Shopify, therefore this app is useless. IT shouldn't even be an app in the first place and should be fully built into Shopify from the jump, allowing for access to form data for external use. Yuck.
The form also likes to glitch and not submit when it feels like it.

Flow also lets you export that data to external systems through something like a HTTP request, giving you another way to send and export the data. Or perhaps exporting to google sheets is a viable option for you.

There is not a "testing" option, so my test form submissions are logged into my account permanently (not a dramatic issue but frustrating to lack control of the data).
Shopify Support was supportive, but is not able to remove them. There are no overrides available.
I was using a different (paid, and should I say not cheap!) solution to solicit subscriptions from visitors to our website/shop until recently. WOW - our daily subscription rate went up by between 5x and 10x when we switched to Shopify Forms. I could only be sorry I did not switch sooner, as we have lost thousands of subscribers in the last 1 year alone... True story!
Good at collecting the data but to reply directly to a form you need to create a completely new email thread? and can't reply directly to the form submission email? Unless i'm wrong then happy to update review!
