App doesn't notify you when you get a form submission. I found a wholesaler request FOUR MONTHS after they'd tried to reach out.
We’ve been using Shopify Forms successfully for a long time, but since February 21 we’ve encountered a serious issue that’s impacting our order processing.
When a customer first signs up via a form (where only first name and email are collected), Shopify creates a customer profile without a last name. Later, when that same customer places an order and enters their full name at checkout, the order contains the correct full name for shipping — but the original customer profile, what is showing in the order tab, is not updated with the last name.
This creates major downstream issues for integrations (in our case, our ERP system), because many systems rely on the customer profile data rather than the order address. As a result, orders fail to sync due to missing last names.
This behavior did not cause issues for us before, so it appears something may have changed recently in how customer data is handled at your side.
For businesses that rely on external systems, this is not a small inconvenience — it directly affects order processing and operations.
We hope this can be addressed quickly, as it forces merchants to add friction (like requiring last name for newsletter signups) just to avoid system failures.

Absolutely ridiculous that the email field type is "text", stopping tracking from working properly. And then support says that this is expected behavior - come on, do better this is basic stuff!
one star is too much , the form never appears on the theme, if there are unsupported themes at least write them down time wasting
How difficult is it to simply make a form builder that you can attach to your footer. You'll spend an hour navigating around this app trying to figure out WHERE or HOW to do this simple thing-- only to draw a blank. Then you'll contact customer support and wait 48 hours (or never).
Very half-assed. UI seems out of date and doesn't line up with the UI/UX of Shopify. Very disappointing considering this is a first party app.
Never like leaving bad reviews, but have to in this case. The limitations in styling and data collection pretty much render this absolutely useless, at least for in-line forms.
If you want to create an obtrusive pop-up form or quick offer it might be ok, but for anything with any level of branding, its a big fat nope! Really disappointing, but it seems to fit Shopify's model of forcing paying customers to pay for yet another third-party app :/
As far as functions go, it's okay - works like all the other forms apps. As far as styling, or the ability to style an inline form, whether in theme or straight up CSS, this is a piece of garbage.
Update: so bad I ditched it for another that has a free version - and that free version is miles more powerful than the best that Shopify Forms can do. Again, what a piece of garbage.
I am not sure why apps made by Shopify always have to be terrible.
I can't uninstall. This app didn't work for me, and there is no cancellation.
