I have just installed this app for my return purpose and I found this app great so far. I contacted their support and they are replying very promptly and without any delay. You can remove their branding from the app for free (conditional) I recommend this app to install in your stores.

Very easy to use Drag and Drop builder. You can create unlimited forms for free. Also you can remove the branding as well.
