Probably one of the best form app to use in shopify and the best thing is it is free and good customer support.

Great app. Easy to setup and configure new forms
Great app - Does what it says it will do. Easy to setup and create forms. I've used it to create a warranty submission form.
I recently started using SK: Advanced Form Builder. I'm finding it very easy to use. It has all the features I need, including check boxes, custom fields, and customizations. But the best part is the great customer support.
