One of the most useful, flexible, and easy to use Form app for Shopify. We have been using it for a long time now for all of our website registrations and other submissions.

The highlight for me is being able to embed it using a simple code which makes it super easy to use via meta-fields. 100% recommended.

Simple to install and use. Feature rich and highly customisable making it easy to match theme style. Can't comment on support as not encountered an problem requiring their help.

SK's Forms is simple and easy to use and perfectly meets my requirements. I have used other form apps before, but the experience is very poor, such as not being able to receive email notifications, not being able to open the app, etc. But SK's form avoids these problems perfectly.
