I'm using this app to create a form. Required some customization on the existing form fields, but that was not part of the app. Developer created fields for me and modified the app as per requirements. Support is quick and respond back positively.

Great plugin! Very easy quick setup and can able to customize the Form appearance with options and possible to add custom CSS also.
