Great and easy app, very simple and nice look to it!

I’ve been using this app to create a form for customers to sign up for product samples, and it has been an absolute breeze! This app is pretty good! It allowed me to build a beautiful custom form easily, and the Klaviyo integration is super helpful.

The only drawback is that you can't customize the template of the customer thank you email after they submit and inquiry- you can only change the text.

Customer support is very responsive as well.

It’s as straightforward as I’d hoped — if I built it myself, this is exactly how I’d want it to be. I did encounter an issue with the form not displaying and couldn’t find a solution in the documentation, but after a quick email exchange, everything was resolved.
