I was stuck with the form not showing, and coudn't find the solution in the documentation, but in an email exange, everything was fixed.

I have been using this form builder app for a short time, but so far the form has made ordering my custom rhinestone items super easy for my clients!

The functionality is very high for a free app. Has many of the features of the more expensive apps but it is all completely free. We use the google sheets integration a lot and find it very helpful. I would recommend learning some basic CSS for styling the forms. There are not many styling tools in the app itself.
